EMPLOYEE BENEFITS OVERVIEW:

Full time regular employees of Number One Vending Service, Inc. (#1) are entitled to the following benefits after a 90 day probationary period.  

Medical insurance is provided by Blue Cross and Blue Shield of Florida Insurance.  The policy is a PPO that offers:

Single Coverage:

•   In network deductable: $1500.00

•   Co-Insurance: 10%

•   Out of Pocket maximum: $3000.00

Family Coverage:

•   In network deductable: $3000.00

•   Co-Insurance: 10%

•   Out of Pocket maximum: $6000.00

#1 will pay 50% of the monthly premium for the employee, and the employee can put dependents on the policy at their own cost.

Dental Insurance is provide by Sun Life Financial.  You can use any dentist that accepts this insurance.

• Annual Deductible - None for preventative services,   $50 per person on basic and major 

• Preventive - paid at 100%

• Basic Procedures - paid at 80%

• Major Procedures - paid at 50%

#1 will pay 50% of the monthly premium for the employee, and the employee can put dependents on the policy at their own cost.

Holidays - Employees will receive pay for holidays if the holiday falls on their regular scheduled work day. These holidays include:

• Memorial Day

• Labor Day

• The Fourth of July

• Thanksgiving Day

• Christmas Day

• New Years Day

Vacation - After one year of full time employment, employees accrue two weeks of paid vacation.
